Logging Symptoms with Your Check-in
Learn how to add physical and mental symptoms to your check-ins and rate their severity.
In addition to your mood, you can log any physical or mental symptoms you are experiencing. The app will suggest common symptoms based on the emotion category you've selected.
Adding Symptoms to Your Check-in
Step 1: On the check-in screen, scroll down to the "Symptoms" section
Step 2: You will see a grid of symptom chips (e.g., "Headache," "Fatigue")
Step 3: Tap on any symptom you are currently experiencing
Step 4: A Severity Sheet will appear for you to rate the symptom

Rating Symptom Severity
After selecting a symptom, you must rate its severity to help track intensity over time.
The Severity Scale
1-2 (Mild):
- Barely noticeable
- Doesn't interfere with activities
- Easy to ignore
- Minor discomfort
3-4 (Mild-Moderate):
- Noticeable but manageable
- Minimal impact on daily activities
- Can still function normally
- Occasional distraction
5-6 (Moderate):
- Clearly noticeable
- Some impact on activities
- May need to adjust plans
- Regular awareness of symptom
7-8 (Moderate-Severe):
- Significant discomfort
- Difficult to ignore
- Impacts ability to function
- May need intervention
9-10 (Severe-Extreme):
- Debilitating
- Unable to perform normal activities
- Requires immediate attention
- Most intense experience
How to Rate Severity
Step 1: When the Severity Sheet appears, you'll see a slider
Step 2: Drag the slider to the number that best represents your symptom intensity
Step 3: Read the description below the slider to confirm it matches your experience
Step 4: Tap "Save" to confirm the severity rating
Adding Multiple Symptoms
You can log as many symptoms as you're experiencing in a single check-in.
Step 1: Rate and save the first symptom
Step 2: Tap on another symptom chip to add it
Step 3: Rate its severity
Step 4: Repeat for all symptoms you want to track
Managing Your Symptom List
View all added symptoms:
- Listed below the symptom grid
- Shows symptom name and severity number
- Color-coded by severity level
Edit a symptom:
- Tap on an already-selected symptom
- Adjust the severity rating
- Save the new rating
Remove a symptom:
- Swipe left on the symptom in your list
- Tap the delete button
- Or tap it again to deselect
Adding Context with Notes
For any extra context about your symptoms:
Step 1: Scroll to the "Add Notes" section
Step 2: Tap to open the text field
Step 3: Type relevant details:
- When the symptom started
- What might have triggered it
- What makes it better or worse
- Any medications taken
- Other relevant context
Step 4: This note is saved with your check-in

For headaches:
- "Started after lunch, might be dehydration"
- "Tension headache, stressful work meeting"
- "Took ibuprofen 30 minutes ago"
For fatigue:
- "Only got 5 hours of sleep last night"
- "Energy crash after skipping breakfast"

For anxiety:
- "Presentation at work in 2 hours"

Step 2: Tap on any past check-in
Step 3: View all symptoms logged with that entry
In 7-Day Summary
The summary card shows:
- Most frequently logged symptoms
- Average severity levels
- Symptom trends over the week
When to Seek Medical Attention
Red Flag Symptoms
Seek immediate help for:
- Chest pain or pressure
- Difficulty breathing
- Severe headache (worst of your life)
- Sudden weakness or numbness
- Confusion or altered consciousness
- Suicidal thoughts
Contact your doctor for:
- Persistent symptoms (lasting weeks)
- Worsening symptoms despite treatment
- New or unusual symptoms

Calibrate your scale:
- 10 should be your worst imaginable
- 1 should be barely noticeable
- Use the same standards each time
- Be honest, not dramatic or minimizing

After logging symptoms for 2-3 weeks, ask your AI assistant to analyze patterns: "What triggers my headaches?" or "When is my fatigue worst?" The AI can identify correlations you might miss.
